1. Employer Tax Treatment
Allowable Business Expense
- The cost of employee health screening or medical check-ups can generally be treated as a deductible business expense if:
- It’s provided wholly and exclusively for business purposes (i.e., to maintain workforce health, productivity, or meet duty of care obligations).
- It’s part of a wider staff welfare or occupational health programme.

Corporation Tax
- The cost is deductible for Corporation Tax purposes, reducing the company’s taxable profits, provided it’s a legitimate staff welfare expense.
2. Employee Benefit in Kind (BIK) Position
Annual Health Screening Exemption
- Under Section 320B ITEPA 2003, one health screening and one medical check-up per year can be provided tax-free to each employee.
- This means:
- The employee does not pay tax or National Insurance on the value of that benefit.
- The company can pay for an annual health screen (including blood tests, health assessments, etc.).
Health Screening = assessment to identify employees at risk of illness or lifestyle disease (e.g., cardiovascular, diabetes, cholesterol).
Medical Check-up = assessment of an employee’s health by a health professional.
⚠️ If more than one screening or check-up per year is provided:
- Any additional screenings may be treated as a taxable benefit, unless they qualify under another exemption (e.g., occupational health requirement).
3. VAT Treatment
- If your business is VAT registered, the VAT on medical screening services may not always be reclaimable, because:
- Most clinical and diagnostic tests are VAT exempt under the “medical care” exemption (VATA 1994, Schedule 9, Group 7)
